Parental "No" Files: iPod Edition

My oldest daughter is 8.

For her birthday this year she told us she wanted an iPod Touch - all her friends had them and she wanted one too. We told her she could choose between all of her closest relatives chipping in and buying her one, or her getting a more traditional and more voluminous haul of gifts.

She chose the iPod.

So what did she show us the other day?

I emailed the developer to ask whether or not it was a licensed McDonald's app, but never did hear back.

In investigating the app, I found dozens of others - all geared towards elementary students. All involved the "fun" use of junk food.

Yes indeed, parents can police a child's apps (though the impact that might have on trust isn't necessarily trivial or easily discountable in the grand scheme of parenting), but what a sad statement that junk food is so normalized that there's literally a genre of apps, specifically targeting kids under 10, that serve only to further glorify dietary crap.

The latest dismal report on world hunger

The United Nations Food and Agriculture Organization (FAO) has just released the latest iteration of its annual report on the State of Food Insecurity in the World 2012.

It’s bottom line estimate: 870 million people in the world are hungry, 852 million of them in developing countries.

The good news is that this figure represents a decline of 132 million people from 1990-92 to 2010-12, or from 18.6 percent to 12.5 percent of the world’s population.  In developing countries, the decline is from 23.2 percent to 14.9 percent.

The not-so-good news: Since 2007-2008, global progress in reducing hunger has slowed and leveled off, and hunger in Africa has gotten worse.

Much of the press attention to the report yawned at the major message but instead focused on errors in the previous estimates, which were higher.

the projections were wrong. They were calculated using figures from non-U.N. sources that were fed into the U.N.’s number-crunching model, because FAO was expected to quickly come up with an estimate of how many people might go hungry from the dual crises of high food prices and the global downturn

The UN bases its hunger projections on figures on population, food supply, food losses, dietary energy requirements, food distribution, and other factors.

The report contains other bad news.  While 870 million people remain hungry, the world confronts a double burden of malnutrition: 1.4 billion people are dealing with the consequences of overweight and obesity.

Focusing on the need to address world hunger Sir Gordon Conway, Professor of International Development, Imperial College, London writes in the Huffington Post:

as I set out in my latest book One billion hungry: can we feed the world?, I believe there is reason for optimism. Yes we can feed the world, but only if we accept that agricultural development is the best route to achieving sustainable economic growth in developing countries, and achieve an agriculture that is highly productive, stable, resilient and equitable.

Sounds like a good plan to me.  Let’s get busy.

But dig the teeniest, tiniest bit and you'll discover that they're just a food industry front group.

So you tell me. Do you think it's ethical and responsible for Concerned Children's Advertisers to have partnered with the Canadian Sugar Institute to create a video that's targeted at teachers to use in their classrooms about "Nature's Sweet Mystery" and healthy eating?

Is Alzheimer’s a Kind of Mental Diabetes?

There seem to be two schools of thought on the relationship of diabetes and Alzheimer’s. The traditional school suspects that diabetes may be a cause, and possibly a significant one, of Alzheimer’s. The newer school hypothesizes that Alzheimer’s is itself actually a kind of diabetes, a Type 3 diabetes, or diabetes of the brain, if you will. The two conditions certainly appear linked in some way: the rate of Alzheimer’s among diabetics is more than double the rate among non-diabetics.

But let’s have some background. Type 1 diabetes is an immune system problem that afflicts only one in 10 diabetics, and is caused primarily by your genes, not your behavior. Type 2 is what hits the other 90 percent, and it is for all practical purposes a disease caused by a high-carbohydrate diet and/or obesity. If Alzheimer’s is indeed a form of diabetes, it may similarly be linked to diet and excess weight.

We already know that statistically, being obese raises one’s odds of impaired brain function. The theory that it might specifically contribute to Alzheimer’s hinges on insulin. Diabetes is the condition wherein your body either fails to produce sufficient insulin, or your cells become insulin-resistant, both of which conditions impede your body’s ability to use sugar as energy, leaving it in your bloodstream where it can cause a variety of problems. What we’ve learned more recently is that when your brain cells become insulin-resistant, things happen that mimic Alzheimer’s to an uncanny degree — confusion, disorientation, memory loss, personality changes, impaired cognition — and that a lack of insulin may be linked to a nasty proteinase plaque that seems to form in the brains of Alzheimer’s victims.

Type 2 diabetes in general, and insulin resistance in particular, are overwhelmingly the result of a diet riddled with sugar and other carbohydrates, mostly in the form of what we call junk food. It’s not hard to connect the dots, and a number of researchers who have done that now speculate that there is a direct causal link between the overconsumption of cheap carbs in junk and processed foods, and mental deterioration, with Alzheimer’s being one possible outcome.

If they’re right, we’ve got one more thing to worry about as a nation, and that is the unprecedented number of Americans who are obese and who will at some point reach the age when dementia and Alzheimer’s manifest themselves. On the other hand, if it can be shown that obesity and the foods that tend to produce it are a primary cause of Alzheimer’s, you’re going to get a lot of people who’ve refused to take their diet and weight seriously suddenly having serious doubts about both.

That’s because Alzheimer’s frankly scares the hell out of a lot of people, many of whom dread it more than death itself. Alzheimer’s is to our mind as cancer is to our body: a terrible wasting affliction, to be avoided even if it means changing our lifestyle somewhat. Alzheimer’s could do to high-carb foods and beverages what cancer did to tobacco: stigmatize it to the point of general revulsion and avoidance. Could all those Doritos and doughnuts and Cokes actually rot your brain? We’re going to find out.

When, And Possibly Why, America Became Obese

We Did Most of the Damage Before Y2K

Why has the American population gotten so fat? That, just in health care terms alone, is literally the $64 billion question. (We’ve come a long way from the days when $64,000 was enough to get people watching a TV show.) There have been enough books, articles, reports, surveys and studies published purporting to have the answer(s) to that question to fuel a very substantial bonfire.

I am not being critical; there were so many different answers floating around out there that I was able to write an entire e-book just summarizing them all, “115 Reasons Why It’s Not Your Fault That You’re Fat.” But some of my “reasons” were strictly for laughs, and many more were applicable to only limited numbers of people, and still others were statistical quirks or associations rather than actual causes of fatness. Even so, there were enough serious possible contributing factors that trying to pin down any of them as primary seemed like a daunting undertaking.

Then I read a recent article by USA Today’s resident health editor, Nanci Hellmich, about rising obesity rates, and where they might be headed, and about halfway through the piece I came across the following:

“(D)ata show that the national obesity rate was relatively stable in the USA from 1960 to 1980, when about 15 percent of people fell into that category. It increased dramatically in the ’80s and ’90s and was up to 32 percent in 2000 and 36 percent in 2010. Because obesity has inched up slightly over the past decade, some experts have speculated that the increase in obesity may be slowing down or leveling off.”

It strikes me that there might be a clue as to the cause of our national overweight epidemic in the fact that the vast majority of it occurred in a 20-year span, when we went from a 15 percent obesity rate in 1980, where it had been for years, to a 32 percent rate in 2000, just 4 percent less than today. Given an obesity rate that more than doubled in those two decades, one might ask what other dramatic and notable cultural or technological changes also occurred in that period. A coincidence isn’t a cause, of course, but sometimes it can lead you to one.

From the dozen or so likeliest subjects in “115 Reasons,” I picked three off the top of my head and did a bit of Googling. Here’s what I came up with.

Fast Foods

I couldn’t nail down the precise growth rate from 1980 to 2000, but in 1970 we were spending just $6 billion a year on fast food, and by 2000 that number had ballooned to over $110 billion, so it had to be fairly heady. More narrowly, in 1980, less than 30 percent of all the food we consumed outside the home was fast food; by the late 90s, it was nearly 40 percent. Even closer to the mark, fast food outlets grew from 109,353 in 1982 to 228,789 in 2002, an increase of 119,436, or 109 percent. Over the same period, the number of full-service restaurants grew from 122,851 to 195,659, an increase of just 72,808, or 59 percent.

It seems safe to say that the number of places to get fast food and the amount of money we spent in them both soared during the critical 1980-2000 years. As suspect causes go, I’d give it an 80 out of 100. But don’t overlook the fact that between fast food and restaurants, we added 200,000 eateries over that period. Both restaurant meals and fast food pack more carbs and calories than home cooking, and we made it vastly more convenient to partake of either kind.

Carbohydrates

Specifically high-calorie sweeteners. According to the U.S. Department of Agriculture, our national intake of carbohydrates was actually experiencing a long-term decline heading into the early 80s, but, “Since that time, the level has increased. This reflects the trend toward increased consumption of grain products and sugars and sweeteners.” Especially sweeteners, and sugar in particular. In 1980, we were putting away about 80 pounds of sugar per American per year, a figure basically unchanged since 1940. By 2000, we were downing 110 pounds of the stuff per capita. When sugar consumption was stable, so was our obesity rate; when the former took off, so did the latter.

I also checked out the history of high fructose corn syrup, which some nutritionists regard as super sugar. Turns out that HFCS-55, the kind used to sweeten countless products, was introduced in the late 1970s. And what do you know: it seems that in 1980, Coca-Cola began sweetening its wares with HFCS. By 2000, it was the most common sweetener in packaged food; possibly the most common single ingredient. I’d have to rate carbs and sweeteners an 87 or so out of 100.

Microwaves

The primary cause of weight gain is excessive eating, and no technology in history has made doing just that so quick and effortless as the now-ubiquitous kitchen zapper. This wasn’t always so, of course. As Wikipedia tells us, “By the late 1970s the technology had improved to the point where prices were falling rapidly. Formerly found only in large industrial applications, microwave ovens increasingly became a standard fixture of most kitchens. By 1986, roughly 25 percent of households in the U.S. owned a microwave oven…” By 2001, it was over 90 percent of American households. Of course, you can use a microwave to cook oatmeal and well as bacon, and to poach salmon as well as to reheat pizza. So the best I score I can give it is a 70.

These three, of course, are all factors that influence our intake of food and especially of fattening food. But there are other plausible candidates in our panel of possible causes, including those that influence our energy expenditure, such as electronic diversions (video games, Facebook, etc.) and sedentary occupations, which have also increased over the years. Not to mention the use of suspect chemicals in agriculture and household products that may have interfered with our metabolisms. Do their growth patterns correspond to the great 1980-2000 weight gain?

I didn’t have time to find out for this column. But I’ll look into it, and get back to you.

American Endocrinologists Declare Obesity a Disease

Regular readers of these pages will be well aware that I am a strong proponent of the ‘medical’ model of obesity.

While I am the first to point out that not all people with adiposity (increased body fat) are necessarily ill (as in individuals with EOSS Stage 0), it is also evident that increased body fat can indeed affect multiple organ systems as well as significantly reduce quality of life.

Thus, while we may quibble about the exact definitions of ‘obesity’ (i.e. versus ‘adiposity’), for those, whose health is affected by excess body fat, it is clear that a medical model better suits their health care needs (irrespective of whether or not this care focuses on weight loss or on generally promoting their health).

Now, the American Association of Clinical Endocrinology (AACE) has released a Position Statement on obesity and obesity medicine, in which they clearly denote obesity as a ‘disease’.

The characterisation of obesity as a disease is based on The American Medical Association’s essential criteria common to all definitions that constitute a disease, namely: (a) an impairment of the normal functioning of some aspect of the body; (b) that has characteristic signs or symptoms; and (c) results in harm or morbidity to the entity affected.

As the position statement points out,

“Obesity meets these 3 conditions. First, obesity is an altered physiological and metabolic state, with environ- mental, genetic, and hormonal determinants, which results in increased morbidity and mortality. Current data provide undeniable evidence for an obesity-centric model of disease with impairment of normal functioning including appetite dysregulation, abnormal energy balance, endo- crine dysfunction including elevated leptin levels and insulin resistance, infertility, dysregulated adipokine signaling, abnormal endothelial function and blood pressure eleva- tion, nonalcoholic fatty liver disease, dyslipidemia, and systemic and adipose tissue inflammation. Some aspects of obesity that lead to an impairment in body function are anatomic and relate to the increase in body fat mass per se such as osteoarthritis, immobility, lymphedema and/or venous stasis, and to some extent sleep apnea. However, critical aspects may also be physiologic, such as the impact of body fat mass on insulin resistance and its associated trait complex with progression to T2DM and cardiovascular disease.”

The statement also notes that although some ‘environmental’ contributors to weight gain and related impact on health may well be under the control of the individual, there is accumulating evidence of environmental factors that may be well beyond the control of affected individuals (e.g. availability and affordability of healthy foods, environmental toxins, endocrine disrupters, but also, sociocultural beliefs and attitudes). On the other hand, there is also clear evidence for the role of genetic susceptibility and a distinct and complex pathophysiology.

Thus, seeing obesity as a simple matter of ‘lifestyle’ choices or simply a ‘cosmetic’ issue, belies both the complex aetiology as well as the very real impacts of this condition on health and functioning.

As the authors point out,

“To say that obesity is not a disease but rather a consequence of chosen lifestyle (i.e., overeating and/or inactivity) is equivalent to saying that lung cancer is also not a disease because it was brought about by volitional cigarette smoking.”

As I have noted in previous posts, the statement describes the important implications of characterizing obesity as a disease state:

“..recognizing obesity as a primary disease state has salient social, political, economic, and transcultural implications. The classification of obesity as a disease will help mobilize society towards the importance of prevention and treatment, and enlist the aid of government, health care providers and payers, and scientific and professional organizations. The disease designation fosters change in attitudes and in the financial support needed for more intensive scientific investigation, drug discovery, resources for patient care, and the development of improved strategies for both prevention and treatment. In the end, it will require a collaborative and coordinated effort by physicians, scientists, pharmaceutical companies, health care payers, government, and patients to mobilize the efforts necessary to combat obesity, ameliorate the suffering of patients, and reduce the social costs of this disease.”

The document also makes important recommendations with regard to medical training and certification of endocrinologists in ‘obesity medicine’:

“AACE proposes development of a certification process for clinical endocrinologists after successful completion of a formalized AACE educational program in obesity medicine. This coordinated education and certification process would focus on the needs and special expertise of the clinical endocrinologist.”

Obviously, given the multitude of individuals affected, obesity management cannot just be the domain of specialists. Indeed, I firmly believe that all health practitioners need to be educated on the the aetiology, prevention and management of obesity - which is exactly why the Canadian Obesity Network’s 5As of Obesity Management were developed for use by all primary care practitioners.

As I have said before, not recognizing obesity (not adiposity!) as a disease, is simply an expression of weight-bias and constitutes nothing less than discrimination of individuals affected by this condition.

Recipe: Mexican Liver and Onions

Need a tasty way to include liver in your diet? Higado Encebollado is a traditional Mexican dish.

Type of dish: Entree, Breakfast
Equipment: Skillet
Servings: Makes 2 to 4 servings

Ingredients:

  • 3/4 pound beef liver, sliced
  • 1 large yellow onion, halved and sliced thin
  • 1 1/2 tsp lard or traditional fat of choice
  • salt
  • 1/2 tsp dried oregano
  • pinch dried rosemary
  • roughly chopped cilantro and sliced avocado for garnish

Directions:

  1. Heat the lard in a large skillet or pot.
  2. Add onion and cook over medium high heat, stirring, until onion is softened.
  3. Reduce heat to low and stir in oregano and rosemary.
  4. Lay the liver slices on top of the onions and cover pot. Let cook for 3 minutes.
  5. Flip the liver slices and this time place them on the pan bottom, covering with the onions. Cover and cook for another 3 minutes.
  6. Stir onions and flip liver one last time, cover and cook for a final 3 minutes.
  7. Check to see if it’s done by slicing one liver in half. It is done if there is no more blood.
  8. Serve sprinkled with cilantro on top and avocado on the side.

"Real fruit in every bite".

Must be good for you, no?

And yet looking at their nutrition fact panels I learned two things.

Firstly that despite there being "fruit in every bite", no bites contain any vitamin C - odd given vitamin C is something I thought was found in most "real" fruit.

Secondly that these are pretty much nutritionally equivalent to Chips-A-Hoy cookies though to be fair they do contain 20% fewer calories and 10% less sugar.

Here's a shopping tip for you: If you put "fruit" in a cookie, it's still a cookie.

Eat cookies because you love cookies, but don't let someone dupe you into thinking you're making a healthful choice or that fruit-inclusive cookies somehow are a "better" cookie choice.

Less bad is not the same as good.

Can Letting Children Under 3 Watch Television Cause Long-Term Damage?

You would be hard-pressed to find anyone willing to say that allowing young children to watch a lot of television is a good idea. However, a new study suggests that it could actually be bad for their health.

Professor Mitch Blair, an officer for health promotion at Royal College of Paediatrics and Child Health, said to the Guardian, “Whether it’s mobile phones, games consoles, TVs or laptops, advances in technology mean children are exposed to screens for longer amounts of time than ever before. We are becoming increasingly concerned, as are paediatricians in several other countries, as to how this affects the rapidly developing brain in children and young people.”

The American Academy of Pediatrics has also suggested guidelines for children under 3, “media — both foreground and background — have potentially negative effects and no known positive effects for children younger than 2 years”.

The study itself goes further by suggesting that even older children should be allowed a maximum of two hours. They also discourage any type of computer or television screen in bedrooms.

It’s not an easy adjustment for parents to make, though. As Justine Roberts, co-founder of Mumsnet in Britain, said, “It would be great if someone could invent a lock that could automatically ensure a daily shut down of all the different devices in and around the home after a designated period. Until such a thing is invented, it’s going to be an ongoing battle to keep on top of everything.”

Hindsight: Endothelial Cell Specific Molecule-1 and Obesity

In 2006, we published a paper in Homone and Metabolic Research, in which we reported on endothelial-cell specific molecule-1 (ESM-1), a molecule which inhibits leukocyte adhesion and migration through the endothelium and also happens to be secreted by fat cells.

In this study we looked at ESM-1 expression and regulation in subcutaneous abdominal adipose tissue samples from 70 postmenopausal women.

In cell culture studies we found that mature adipocytes produced more ESM-1 than preadipocytes. We also found that insulin and cortisol inhibited adipocyte ESM-1 production. This inhibitory effect of insulin was attenuated by insulin resistance, as ESM-1 gene expression in subcutaneous adipose tissue was increased in obese, hyperinsulinemic women.

On the other hand, modest 5% weight loss in 14 women did not markedly change gene expression. Circulating ESM-1 levels increased significantly, albeit modestly.

Thus, we confirmed that ESM-1 is actively produced by adipocytes but that circulating ESM-1 levels are reduced in the overweight and obese, consistent with the notion that ESM-1 may play some role in obesity-associated vascular disease.

This clearly did not turn out to be a hotbed of adipocyte research - according to Google Scholar, this paper has only been cited 13 times.
